Do I have to get all of the forms notarized?

The Transfer Form labelled "PHILADELPHIA EAGLES SBL TRANSFER REQUEST FORM - NEW SBL HOLDER ("TRANSFEREE")" has a section at the bottom of the page for a Notary and is the only page that needs to be notarized.
